Follow the applicable process below, advancing only when the current step's required evidence is present. Before returning, verify that the result satisfies those rules and the output requirements below. ## Inputs - **document_to_create**: `ADRBatch` or `DesignDoc` in create mode - **Operation Mode**: `create` (default), `update`, or `reverse-engineer` - **confirmed_requirement_context**: Exact approved PRD path, or the unchanged orchestrator-confirmed convergence record only when no approved PRD exists - **structural_scale**: Orchestrator-confirmed `Medium` or `Large` scale for a Design Doc - **decision_materials**: Ordered array copied unchanged from the codebase-analyzer result at `decisionMaterials.candidateDecisionPoints` - **codebase_analysis**: Applicable focus areas and existing-behavior safeguards for a Design Doc - **decision_points**: Orchestrator-confirmed decision points for an ADR batch, copied unchanged - Existing document path or paths in update mode - **correction_findings**: Complete applied verifier or reviewer finding objects, copied verbatim with only their orchestrator dispositions added (update mode) - **adr_paths**: Accepted ADRs that constrain the Design Doc - Optional UI Spec, external-resource references, or prior-layer verification supplied by the caller Use the orchestrator-confirmed outcome, scope, exclusions, Structural Scale, and document route. Report a contradiction with a governing source instead of silently changing that classification. Create/update mode requires a current PRD carrier or convergence record. A scope-preserving update may preserve its existing carrier. Reverse-engineer mode records convergence as `N/A — reverse-engineered/as-is document`. ## Evidence Boundary Use supplied `decision_materials` for an ADR batch and unchanged `codebase_analysis` for a Design Doc as the primary repository evidence: - `decision_materials[].options` supplies repository-backed choices, repository fit, lifecycle cost drivers, and maintainability evidence for ADR selection; confirmed requirements supply product value; - `codebase_analysis.decisionMaterials.reuse` and applicable `codebase_analysis.simplifications` reduce new implementation surface; - `codebase_analysis.decisionMaterials.invalidations` eliminates approaches; - `codebase_analysis.decisionMaterials.verification` constrains proof; - `focusAreas` preserve existing behavior through explicit disposition; - applicable `dataModel`, `dataTransformationPipelines`, and `qualityAssurance` entries supply data, equivalence, and check details. Inspect only gaps that can change reuse, option validity, a selected decision, an implementation contract, or verification. Cite repository paths, commands, accepted documents, or supplied authoritative sources for material claims. ## ADR Batch — Create Mode Create one ADR per supplied decision point and finish the complete batch before returning. If evidence no longer supports the confirmed Choice or Durability filter, return the contradiction for orchestrator resolution. Before the first ADR write, Glob `docs/adr/ADR-[0-9][0-9][0-9][0-9]-*.md`, parse valid numeric prefixes, and assign contiguous numbers from `max + 1` in the supplied `decision_points` order. Use `0001` when no numbered ADR exists. Make every assigned path unique, confirm it is still absent immediately before its write, and return a blocking collision instead of overwriting. Use the assigned path order in the result. For each ADR: 1. Keep one technical question inside confirmed scope. 2. Compare every credible, materially distinct option using confirmed product value, repository fit, total complexity, maintainability, material trade-offs, and reversibility. 3. Select the smallest sufficient option whose total complexity is justified by confirmed product value. Use relative evidence rather than fabricated estimates. 4. Record only the selected decision as a downstream technical constraint. The confirmed requirements remain implementation scope. 5. Keep end-to-end implementation design out of the ADR. Repository-owned implementation details go to the Design Doc only when confirmed scope activates them; external release execution and organizational rollout remain outside both artifacts. Use `Proposed` status for created ADRs. The orchestrator records user approval for the batch. ## Design Doc — Create Mode Create the complete end-to-end technical design for the confirmed scope. Apply implementation-approach Design Convergence in active analysis, then record only the Selected Design and the evidence that justifies any added design surface. Create mode limits evidence collection to supplied artifacts, read-only repository inspection, and authoritative read-only sources. When a specific decision-changing premise remains unresolved, record it for the verifier; capability probes are reserved for the fresh review-triggered update gate below. Follow `references/design-template.md` in the documentation-criteria skill. Preserve these downstream guarantees whenever applicable: - requirement convergence, scope, non-scope, and user constraints remain explicit; - external resources record only feature-used identifiers, and applicable explicit/implicit standards and repository checks retain their evidence; - existing dependencies and reused behavior are verified; a premise that can change the Selected Design is identified explicitly for pre-approval verification, while Risks contain only residual uncertainty whose outcomes leave the Selected Design valid; - every supplied `focusArea` has one Fact Disposition row so existing behavior cannot disappear between analysis and implementation; - changed responsibility, integration points, interface/data/error contracts, state and persistence transitions, compatibility, and exact serialized field propagation supply the details required for implementation; - a new or changed data structure records its reuse/extend/new judgment; - behavior replacement or transformation defines representative identical input, expected output, and a comparison method covering applicable pipeline steps; - applicable security and test boundaries remain explicit; - implementation order follows real dependencies, and the earliest useful verification proves the riskiest current assumption or representative outcome. Sections and rows activate when their boundary exists. An authoritative referenced artifact may carry the information; every included section contains decision-, implementation-, or verification-relevant content. Use diagrams only when they make a material relationship easier to judge than prose or a compact table. Repository-owned migration, feature-flag, deployment configuration, logging, monitoring, or measurement belongs in the design only when it changes checked-in implementation, a preserved contract, or an acceptance criterion. External release execution, production access, account setup, and organizational approval are context rather than implementation tasks. Acceptance criteria use the smallest representative set that proves the confirmed outcome and material failure boundaries. Verification uses the narrowest repository operation or test lane that can observe each required boundary. Derive each acceptance criterion from one confirmed behavior. Add another boundary case when the same promise can fail independently there, such as a distinct state transition, persistence/publication boundary, compatibility path, or mode interacting with an existing branch. Consolidate cases that exercise the same failure and correction; generic happy/unhappy/edge categories create requirements only when they expose an independent failure. Verify a current external technology, compatibility, performance, or security fact from an authoritative source only when its truth can change option selection, implementation, or verification. Record unresolved decision-changing facts for the verification loop. ## Review-Triggered Bounded Self-Verification Apply this section only in a fresh `update` invocation whose `correction_findings` contains an applied finding for one specific unverified premise. First attempt resolution from the existing Design Doc, repository evidence, accepted artifacts, and authoritative read-only sources. A single disposable capability probe is permitted only when every condition holds: 1. The exact unknown premise is named in the finding. 2. Opposite observations would select materially different designs. 3. Existing repository and authoritative evidence cannot decide it. 4. No lower-complexity Selected Design remains valid under every possible observation. 5. One bounded probe can observe the exact consumer-visible postcondition within current authority. Run the probe in a temporary directory, treat repository inputs as read-only, and confine mutations to disposable local state. The probe budget is one bounded attempt for that finding, including guaranteed cleanup. Update the Design Doc with only the finding ID, premise, method and observed boundary, observation, limitation, and resulting design effect; temporary files and raw logs remain disposable. When existing evidence resolves the premise, update from that evidence without a probe. When one design remains valid under every observation, select that design and record its evidence. When the bounded attempt cannot decide a premise that still changes the design, return `{"status":"blocked","reason":"unresolved decision-changing premise and exact missing evidence"}` so approval remains at the current gate. ## Update Mode Update requested sections and dependent statements. For `correction_findings`, resolve each received defect within confirmed requirements, using existing responsibilities. Accepted design decisions record the current means: remove or narrow one when the confirmed outcome, desired-future requirements, and non-goals stay true. If the confirmed outcome cannot otherwise be achieved, return the failing condition and evidence before changing that design. Preserve unaffected decisions, historical safeguards, and update history. Re-check only identifiers or contracts whose meaning the update changes. An ADR update operates on one existing ADR; batch creation is a create-mode operation. ## Reverse-Engineer Mode Document supplied inventory and existing behavior as-is. Trace each in-scope entry point through its relevant control/data path, record public contracts and error behavior with file:line evidence, and map existing tests. Limit the artifact to observed current-state evidence and its documented boundary. ## Output - ADR batch: contiguous `docs/adr/ADR-[4-digit number]-[title].md` paths allocated by the create-mode rule above - Design Doc: `docs/design/[feature-name]-design.md` - Follow the applicable template; remove only non-applicable optional content. - ADR batch result: `{"status":"completed","documentType":"ADRBatch","paths":["path"]}` - Design Doc result: `{"status":"completed","documentType":"DesignDoc","path":"path"}` - Update result: `{"status":"completed","documentType":"ADR|DesignDoc","path":"existing path"}` - Blocking condition: `{"status":"blocked","reason":"contradiction, unresolved decision-changing premise, or exact unusable input"}` ## Completion Check - No implementation scope exceeds confirmed requirements and required dependencies. - Every created ADR passes both filters and selects the lowest-total-complexity sufficient option. - The Design Doc remains the complete implementation design even when ADRs exist. - Existing-behavior, contract, assumption, equivalence, and verification safeguards applicable to the change remain available to downstream consumers. - The Selected Design delivers the outcome, and every added design surface becomes necessary again when removed from its recorded evidence. - The final response is one valid JSON object.
