    - Unpinned Action versions — all Actions need full 40-char SHA pins. Risk tiers (ascending): `@v4` (mutable tag), `@main`/`@master` (branch ref, worst), `@latest`. Correct form: `uses: actions/checkout@<40-char-SHA>  # vN`; resolve via `gh api repos/actions/checkout/commits/<tag> --jq .sha`. Severity: **high** for version tags, **critical** for branch refs; no downgrade for first-party Actions.
    - Short SHAs (fewer than 40 hex chars, e.g. `@abc1234`) — treat as unpinned; short SHAs can collide, not cryptographically safe; always full 40-char commit SHA
    - All tests in single large job when parallelism available
    - Skipping `fail-fast: false` — early exit hides failures in other matrix cells
    - Hard-coded Python versions, no matrix — always test ≥ 2 versions
    - `pip install .` without lockfile — non-reproducible; use `uv sync` or pinned requirements
    - `actions/cache` placed after steps it should accelerate — cache restore runs at step execution time; cache step last → restore never fires, only post-step save; cache useless that run
    - `workflow_dispatch` as only trigger — always include `push: branches: [main]` + `pull_request` so CI runs automatically; `workflow_dispatch`-only = CI never blocks PR merge
    - Secrets in workflow env without GitHub Secrets (e.g. `env: API_KEY: "hardcoded-value"` or `env: API_KEY: ${{ env.API_KEY }}` sourced from committed file) — always `${{ secrets.MY_SECRET }}`; hardcoded secrets visible in run logs + git history
    - Matrix values declared but never consumed — e.g. `matrix.version` defined but no `actions/setup-<lang>` reads it; declared versions have no effect, runner uses pre-installed
    - `runs-on` hardcoded when `matrix.os` declared — same failure as unconsumed matrix values: OS dimension silently ignored, one OS ever tested. Flag as **primary** finding (high severity), not additional observation. Fix: `runs-on: ${{ matrix.os }}`.

Multi-cycle CI remediation (diagnose → fix → verify → close, Dependabot triage backlog) reports its phases in the return envelope; the dispatching skill owns the task state.
