This skill grounds those conversations in the real diagnostic framework of\nthe DSM-5-TR (text revision) so that \"could this be X?\" becomes a structured\ncomparison — symptoms weighed against actual criteria, with met, unmet, and unknown\nkept separate — instead of a guess. It was built from a copy of the DSM-5-TR text\nrevision and organized as a searchable reference library, with safety handling and\ncalibrated language built into every answer.
\n
Who it is for: clinicians and practitioners double-checking criteria, specifiers, or\ndifferentials; patients and family members who want an evidence-based understanding\nof a condition and good questions to take to a provider; and anyone who needs to\nexplain a mental health topic accurately and without stigma.
\n
What it is not: it is not a diagnostic tool, it does not render \"you have X\"\nverdicts, it gives no treatment or medication advice, and it is not the manual\nitself. It is a paraphrased companion for orientation and education, and it always\nsays so.
\n
What You Get
\n
\n
\n
\n
Item
\n
What it provides
\n
\n
\n
\n
\n

SKILL.md

\n
The conversation workflow: safety triage first, question/audience clarification, reference routing, criteria comparison, differential reasoning, and calibrated communication.
\n
\n
\n

references/
(66 files)
\n
Chapter indexes plus part files: foundation (00–02), per-chapter references for all 22 DSM-5-TR diagnostic classes (10–31), Part III (32–33), and cross-cutting differentials (40). Large chapters are split into an index plus focused part files so any agent can read a file in one call.
\n
\n
\n

scripts/lookup.py

\n
A keyword search across every reference file — finds where a topic lives and which files to read first.
\n
\n
\n

evals/evals.json

\n
Output-quality test cases that keep the skill's answers honest across safety, criteria fidelity, and plain-language scenarios.
\n
\n
\n
\n
Quick Start
\n
No setup, no API keys, no dependencies. Two ways to use it:
\n
\n

Browse by chapter. Open
references/
and pick the file for the condition in\nquestion (for example,
13-depressive-disorders.md
for depression).
\n
\n

Search by keyword. Find where any topic lives:
\n

python3 scripts/lookup.py 'insomnia'\n

\n
The script prints matching lines grouped by file and a short list of the best\nreference files to read. Add
--list
to see every reference file with its\none-line description, or
--json
for machine-readable output.
\n
\n
\n
Triggers
\n
Load this skill when someone asks about:
\n
\n
What a set of symptoms \"could be\" or whether a description matches a known\ncondition.
\n
Diagnostic criteria, specifiers, or codes for a mental health condition.
\n
The difference between two conditions (for example, ADHD vs. anxiety, or delirium\nvs. dementia).
\n
How common a condition is, when it starts, or how it typically runs its course.
\n
A plain-language explanation of a diagnosis for a patient or family member.
\n
What questions to bring to a clinician or therapist.
\n
\n
Requirements
\n
\n
Python 3 (standard library only) if you want to use
scripts/lookup.py
.
\n
The reference files, which ship with the skill — no downloads needed.
\n
For exact verbatim criteria, codes, and recording procedures, consult the official\nDSM-5-TR (the user's copy). This skill is a companion, not the authoritative text.
\n
\n
DSM-5-TR is © American Psychiatric Association (2022). This skill is an independent\ncompanion reference; it paraphrases and summarizes the manual for orientation and\neducation and is not a substitute for the manual, for clinical training, or for\nprofessional evaluation. Always verify criteria and codes against the official\nDSM-5-TR before formal use.
\n","files":[{"path":"evals/evals.json","sizeBytes":16721,"isText":true},{"path":"README.md","sizeBytes":3902,"isText":true},{"path":"references/00-overview-and-method.md","sizeBytes":19894,"isText":true},{"path":"references/01-safety-and-boundaries.md","sizeBytes":14460,"isText":true},{"path":"references/02-assessment-and-differential.md","sizeBytes":20440,"isText":true},{"path":"references/10-neurodevelopmental-adhd-and-learning-disorders.md","sizeBytes":33613,"isText":true},{"path":"references/10-neurodevelopmental-autism-spectrum-disorder.md","sizeBytes":19608,"isText":true},{"path":"references/10-neurodevelopmental-communication-disorders.md","sizeBytes":25018,"isText":true},{"path":"references/10-neurodevelopmental-disorders.md","sizeBytes":7066,"isText":true},{"path":"references/10-neurodevelopmental-intellectual-developmental-disorders.md","sizeBytes":17791,"isText":true},{"path":"references/10-neurodevelopmental-motor-and-tic-disorders.md","sizeBytes":32810,"isText":true},{"path":"references/11-schizophrenia-spectrum-and-other-psychotic.md","sizeBytes":6117,"isText":true},{"path":"references/11-schizophrenia-spectrum-psychotic-disorders.md","sizeBytes":27800,"isText":true},{"path":"references/11-schizophrenia-spectrum-schizoaffective-catatonia-and-other.md","sizeBytes":30642,"isText":true},{"path":"references/12-bipolar-and-related-disorders.md","sizeBytes":4239,"isText":true},{"path":"references/12-bipolar-cyclothymic-and-other.md","sizeBytes":30048,"isText":true},{"path":"references/12-bipolar-episodes-and-disorders.md","sizeBytes":28948,"isText":true},{"path":"references/13-depressive-disorders.md","sizeBytes":5308,"isText":true},{"path":"references/13-depressive-dmdd-and-mdd.md","sizeBytes":18227,"isText":true},{"path":"references/13-depressive-other-and-specifiers.md","sizeBytes":6942,"isText":true},{"path":"references/13-depressive-persistent-pmdd-and-etiological.md","sizeBytes":24083,"isText":true},{"path":"references/14-anxiety-agoraphobia-gad-and-other.md","sizeBytes":24487,"isText":true},{"path":"references/14-anxiety-disorders.md","sizeBytes":4290,"isText":true},{"path":"references/14-anxiety-phobias-panic-and-separation.md","sizeBytes":33178,"isText":true},{"path":"references/15-obsessive-compulsive-and-related-disorders.md","sizeBytes":4705,"isText":true},{"path":"references/15-ocd-bdd-and-hoarding.md","sizeBytes":30287,"isText":true},{"path":"references/15-ocd-body-focused-and-other.md","sizeBytes":23735,"isText":true},{"path":"references/16-trauma-acute-adjustment-and-grief.md","sizeBytes":25857,"isText":true},{"path":"references/16-trauma-and-stressor-related-disorders.md","sizeBytes":4519,"isText":true},{"path":"references/16-trauma-attachment-and-ptsd.md","sizeBytes":30393,"isText":true},{"path":"references/17-dissociative-disorders.md","sizeBytes":41197,"isText":true},{"path":"references/18-somatic-symptom-and-related-disorders.md","sizeBytes":43965,"isText":true},{"path":"references/19-feeding-and-eating-disorders.md","sizeBytes":4725,"isText":true},{"path":"references/19-feeding-eating-anorexia-bulimia-and-bed.md","sizeBytes":27820,"isText":true},{"path":"references/19-feeding-eating-arfind-pica-rumination-and-other.md","sizeBytes":23681,"isText":true},{"path":"references/20-elimination-disorders.md","sizeBytes":20060,"isText":true},{"path":"references/21-sleep-wake-circadian-parasomnias-and-movement.md","sizeBytes":36448,"isText":true},{"path":"references/21-sleep-wake-disorders.md","sizeBytes":7434,"isText":true},{"path":"references/21-sleep-wake-insomnia-hypersomnia-narcolepsy-breathing.md","sizeBytes":33256,"isText":true},{"path":"references/21-sleep-wake-substance-medication-induced-and-other.md","sizeBytes":10932,"isText":true},{"path":"references/22-sexual-dysfunctions-female-and-other.md","sizeBytes":24977,"isText":true},{"path":"references/22-sexual-dysfunctions-male-disorders.md","sizeBytes":20821,"isText":true},{"path":"references/22-sexual-dysfunctions.md","sizeBytes":6366,"isText":true},{"path":"references/23-gender-dysphoria.md","sizeBytes":22644,"isText":true},{"path":"references/24-disruptive-impulse-control-and-conduct-disorders.md","sizeBytes":36441,"isText":true},{"path":"references/25-substance-caffeine-cannabis-hallucinogens-inhalants-opioids-sedatives.md","sizeBytes":39212,"isText":true},{"path":"references/25-substance-framework-alcohol.md","sizeBytes":26978,"isText":true},{"path":"references/25-substance-related-and-addictive-disorders.md","sizeBytes":6283,"isText":true},{"path":"references/25-substance-stimulants-tobacco-other-gambling.md","sizeBytes":34900,"isText":true},{"path":"references/26-neurocognitive-delirium-and-syndromes.md","sizeBytes":30242,"isText":true},{"path":"references/26-neurocognitive-differential-and-guidance.md","sizeBytes":19021,"isText":true},{"path":"references/26-neurocognitive-disorders.md","sizeBytes":8366,"isText":true},{"path":"references/26-neurocognitive-etiologic-subtypes.md","sizeBytes":36029,"isText":true},{"p
